md: don't return 0 from array_state_store
Returning zero from a 'store' function is bad.
The return value should be either len length of the string
or an error.
So use 'len' if 'err' is zero.

dmaengine: Fix choppy sound because of unimplemented resume
Some drivers implement only pause operation (no resuming). Example is
pl330 where pause is needed for getting residuum. pl330 does not support
resume operation, transfer must be stopped after pause.
However for slaves this is exposed always as "pause and resume" which
introduces subtle errors on Odroid U3 board (Exynos4412 with pl330).
After adding pause function to pl330 driver the audio playback
(utilizing DMA) gets choppy after some time (approximately 24 hours).
Fix this by exposing "cmd_pause" if and only if pause and resume are
implemented.

ALSA: hda - Abort the probe without i915 binding for HSW/BDW
The previous patch tried to continue the probe if i915 binding fails.
For for simplicity reason, we haven't implemented abort even for
controller chips that are dedicated for HDMI/DP on HSW and BDW.
However, Mengdong suggested that this can be dangerous; BIOS may
disable gfx power well although the PCI entry for HD-audio is left,
and this may result in the unexpected behavior, kernel errors, etc.
For avoiding this situation, abort the probe at i915 binding failure
only for HSW/BDW chips selectively. For other chips, it still
continues.

net: don't wait for order-3 page allocation
We saw excessive direct memory compaction triggered by skb_page_frag_refill.
This causes performance issues and add latency. Commit
5640f7685831e0
introduces the order-3 allocation. According to the changelog, the order-3
allocation isn't a must-have but to improve performance. But direct memory
compaction has high overhead. The benefit of order-3 allocation can't
compensate the overhead of direct memory compaction.
This patch makes the order-3 page allocation atomic. If there is no memory
pressure and memory isn't fragmented, the alloction will still success, so we
don't sacrifice the order-3 benefit here. If the atomic allocation fails,
direct memory compaction will not be triggered, skb_page_frag_refill will
fallback to order-0 immediately, hence the direct memory compaction overhead is
avoided. In the allocation failure case, kswapd is waken up and doing
compaction, so chances are allocation could success next time.
alloc_skb_with_frags is the same.
The mellanox driver does similar thing, if this is accepted, we must fix
the driver too.

net: igb: fix the start time for periodic output signals
When programming the start of a periodic output, the code wrongly places
the seconds value into the "low" register and the nanoseconds into the
"high" register. Even though this is backwards, it slipped through my
testing, because the re-arming code in the interrupt service routine is
correct, and the signal does appear starting with the second edge.
This patch fixes the issue by programming the registers correctly.

drm/radeon: fix freeze for laptop with Turks/Thames GPU.
Laptop with Turks/Thames GPU will freeze if dpm is enabled. It seems
the SMC engine is relying on some state inside the CP engine. CP needs
to chew at least one packet for it to get in good state for dynamic
power management.
This patch simply disabled and re-enable DPM after the ring test which
is enough to avoid the freeze.

ntb: initialize max_mw for Atom before using it
Commit
ab760a0 (ntb: Adding split BAR support for Haswell platforms)
changed ntb_device's mw from a fixed-size array into a pointer that is
allocated based on limits.max_mw; however, on Atom platforms, max_mw
is not initialized until ntb_device_setup(), which happens after the
allocation.
Fill out max_mw in ntb_atom_detect() to match ntb_xeon_detect(); this
happens before the use of max_mw in the ndev->mw allocation.
Fixes a null pointer dereference on Atom platforms with ntb hardware.

ALSA: hda - Re-add the lost fake mute support
Yet another regression by the transition to regmap cache; for better
usability, we had the fake mute control using the zero amp value for
Conexant codecs, and this was forgotten in the new hda core code.
Since the bits 4-7 are unused for the amp registers (as we follow the
syntax of AMP_GET verb), the bit 4 is now used to indicate the fake
mute. For setting this flag, snd_hda_codec_amp_update() becomes a
function from a simple macro. The bonus is that it gained a proper
function description.

enic: fix memory leak in rq_clean
When incoming packet qualifies for rx_copybreak, we copy the data to newly
allocated skb. We do not free/unmap the original buffer. At this point driver
assumes this buffer is unallocated. When enic_rq_alloc_buf() is called for
buffer allocation, it checks if buf->os_buf is NULL. If its not NULL that means
buffer can be re-used.
When vnic_rq_clean() is called for freeing all rq buffers, and if the
rx_copybreak reused buffer falls outside the used desc, we do not free the
buffer. The following trace is observer when dma-debug is enabled.
Fix is to walk through complete ring and clean if buffer is present.

We do not check the return value of enic_dev_stats_dump(). If allocation
fails, we will hit NULL pointer reference.
Return only if memory allocation fails. For other failures, we return the
previously recorded values.

enic: unlock napi busy poll before unmasking intr
There is a small window between vnic_intr_unmask() and enic_poll_unlock_napi().
In this window if an irq occurs and napi is scheduled on different cpu, it tries
to acquire enic_poll_lock_napi() and hits the following WARN_ON message.
Fix is to unlock napi_poll before unmasking the interrupt.

bridge: fix multicast router rlist endless loop
Since the addition of sysfs multicast router support if one set
multicast_router to "2" more than once, then the port would be added to
the hlist every time and could end up linking to itself and thus causing an
endless loop for rlist walkers.
So to reproduce just do:
echo 2 > multicast_router; echo 2 > multicast_router;
in a bridge port and let some igmp traffic flow, for me it hangs up
in br_multicast_flood().
Fix this by adding a check in br_multicast_add_router() if the port is
already linked.
The reason this didn't happen before the addition of multicast_router
sysfs entries is because there's a !hlist_unhashed check that prevents
it.

tipc: disconnect socket directly after probe failure
If the TIPC connection timer expires in a probing state, a
self abort message is supposed to be generated and delivered
to the local socket. This is currently broken, and the abort
message is actually sent out to the peer node with invalid
addressing information. This will cause the link to enter
a constant retransmission state and eventually reset.
We fix this by removing the self-abort message creation and
tear down connection immediately instead.

ALSA: hda - Continue probing even if i915 binding fails
Currently snd-hda-intel driver aborts the probing of Intel HD-audio
controller with i915 power well management when binding with i915
driver via hda_i915_init() fails. This is no big problem for Haswell
and Broadwell where the HD-audio controllers are dedicated to
HDMI/DP, thus i915 link is mandatory. However, Skylake, Baytrail and
Braswell have only one controller and both HDMI/DP and analog codecs
share the same bus. Thus, even if HDMI/DP isn't usable, we should
keep the controller working for other codecs.
For fixing this, this patch simply allows continuing the probing even
if hda_i915_init() call fails. This may leave stale sound components
for HDMI/DP devices that are unbound with graphics. We could abort
the probing selectively, but from the code simplicity POV, it's better
to continue in all cases.

In this case it was triggered by running tcpdump but it's not necessary
reproducible on all systems.

Compound pages cannot be migrated and it was not expected that such pages
be marked for NUMA balancing. This did not take into account that drivers
such as net/packet/af_packet.c may insert compound pages into userspace
with vm_insert_page. This patch tells the NUMA balancing protection
scanner to skip all VM_MIXEDMAP mappings which avoids the possibility that
compound pages are marked for migration.

If zs_create_pool()->create_handle_cache()->kmem_cache_create() or
pool->name allocation fails, zs_create_pool()->destroy_handle_cache()
will dereference the NULL pool->handle_cachep.
Modify destroy_handle_cache() to avoid this.

mm: memcontrol: fix false-positive VM_BUG_ON() on -rt
On -rt, the VM_BUG_ON(!irqs_disabled()) triggers inside the memcg
swapout path because the spin_lock_irq(&mapping->tree_lock) in the
caller doesn't actually disable the hardware interrupts - which is fine,
because on -rt the tophalves run in process context and so we are still
safe from preemption while updating the statistics.
Remove the VM_BUG_ON() but keep the comment of what we rely on.

Reproduce method (re-add a node)::
Hot-add nodeA --> remove nodeA --> hot-add nodeA (panic)
This seems an use-after-free problem, and the root cause is
zone->wait_table was not set to *NULL* after free it in
try_offline_node.
When hot re-add a node, we will reuse the pgdat of it, so does the zone
struct, and when add pages to the target zone, it will init the zone
first (including the wait_table) if the zone is not initialized. The
judgement of zone initialized is based on zone->wait_table:

so if we do not set the zone->wait_table to *NULL* after free it, the
memory hotplug routine will skip the init of new zone when hot re-add
the node, and the wait_table still points to the freed memory, then we

people after the i/o operation with the page is done, such as mentioned
above.

cpufreq: Remove cpufreq_update_policy()
cpufreq_update_policy() was kept as a separate routine earlier as it was
handling migration of sysfs directories, which isn't the case anymore.
It is only updating policy->cpu now and is called by a single caller.
The WARN_ON() isn't really required anymore, as we are just updating the
cpu now, not moving the sysfs directories.

cpufreq: Restart governor as soon as possible
__cpufreq_remove_dev_finish() is doing two things today:
- Restarts the governor if some CPUs from concerned policy are still
online.
- Frees the policy if all CPUs are offline.
The first task of restarting the governor can be moved to
__cpufreq_remove_dev_prepare() to restart the governor early. There is
no race between _prepare() and _finish() as they would be handling
completely different cases. _finish() will only be required if we are
going to free the policy and that has nothing to do with restarting the
governor.

cpufreq: Initialize policy->kobj while allocating policy
policy->kobj is required to be initialized once in the lifetime of a
policy. Currently we are initializing it from __cpufreq_add_dev() and
that doesn't look to be the best place for doing so as we have to do
this on special cases (like: !recover_policy).
We can initialize it from a more obvious place cpufreq_policy_alloc()
and that will make code look cleaner, specially the error handling part.
The error handling part of __cpufreq_add_dev() was doing almost the same
thing while recover_policy is true or false. Fix that as well by always
calling cpufreq_policy_put_kobj() with an additional parameter to skip
notification part of it.

cpufreq: Stop migrating sysfs files on hotplug
When we hot-unplug a cpu, we remove its sysfs cpufreq directory and if
the outgoing cpu was the owner of policy->kobj earlier then we migrate
the sysfs directory to under another online cpu.
There are few disadvantages this brings:
- Code Complexity
- Slower hotplug/suspend/resume
- sysfs file permissions are reset after all policy->cpus are offlined
- CPUFreq stats history lost after all policy->cpus are offlined
- Special management of sysfs stuff during suspend/resume
To overcome these, this patch modifies the way sysfs directories are
managed:
- Select sysfs kobjects owner while initializing policy and don't change
it during hotplugs. Track it with kobj_cpu created earlier.
- Create symlinks for all related CPUs (can be offline) instead of
affected CPUs on policy initialization and remove them only when the
policy is freed.
- Free policy structure only on the removal of cpufreq-driver and not
during hotplug/suspend/resume, detected by checking 'struct
subsys_interface *' (Valid only when called from
subsys_interface_unregister() while unregistering driver).
Apart from this, special care is taken to handle physical hoplug of CPUs
as we wouldn't remove sysfs links or remove policies on logical
hotplugs. Physical hotplug happens in the following sequence.
Hot removal:
- CPU is offlined first, ~ 'echo 0 >
/sys/devices/system/cpu/cpuX/online'
- Then its device is removed along with all sysfs files, cpufreq core
notified with cpufreq_remove_dev() callback from subsys-interface..
Hot addition:
- First the device along with its sysfs files is added, cpufreq core
notified with cpufreq_add_dev() callback from subsys-interface..
- CPU is onlined, ~ 'echo 1 > /sys/devices/system/cpu/cpuX/online'
We call the same routines with both hotplug and subsys callbacks, and we
sense physical hotplug with cpu_offline() check in subsys callback. We
can handle most of the stuff with regular hotplug callback paths and
add/remove cpufreq sysfs links or free policy from subsys callbacks.

ACPI / init: Switch over platform to the ACPI mode later
Commit
73f7d1ca3263 "ACPI / init: Run acpi_early_init() before
timekeeping_init()" moved the ACPI subsystem initialization,
including the ACPI mode enabling, to an earlier point in the
initialization sequence, to allow the timekeeping subsystem
use ACPI early. Unfortunately, that resulted in boot regressions
on some systems and the early ACPI initialization was moved toward
its original position in the kernel initialization code by commit
c4e1acbb35e4 "ACPI / init: Invoke early ACPI initialization later".
However, that turns out to be insufficient, as boot is still broken
on the Tyan S8812 mainboard.
To fix that issue, split the ACPI early initialization code into
two pieces so the majority of it still located in acpi_early_init()
and the part switching over the platform into the ACPI mode goes into
a new function, acpi_subsystem_init(), executed at the original early
ACPI initialization spot.
That fixes the Tyan S8812 boot problem, but still allows ACPI
tables to be loaded earlier which is useful to the EFI code in
efi_enter_virtual_mode().

score: Fix exception handler label
The latest version of modinfo fails to compile score architecture
targets with the following error.
FATAL: The relocation at __ex_table+0x634 references
section "__ex_table" which is not executable, IOW
the kernel will fault if it ever tries to
jump to it. Something is seriously wrong
and should be fixed.
The probem is caused by a bad label in an __ex_table entry.

MIPS: Loongson: Do not register 8250 platform device from module.
If CONFIG_SERIAL_8250 is set to m, the Loongson seria.ko module might get
unloaded while the serial driver modules are still loaded resulting in
stale references to the destroyed platform_device instance.
Anyway, platform devices should always be registered indicated what
devices are present, _not_ what drivers have been configured.

ALSA: hda - Don't actually write registers for caps overwrites
Along with the transition to regmap for managing the cached parameter
reads, the caps overwrite was also moved to regmap cache. The cache
change itself works, but it still tries to write the non-existing verb
(the HDA parameter is read-only) wrongly. It's harmless in most
cases, but some chips are picky and may result in the codec
communication stall.
This patch avoids it just by adding the missing flag check in
reg_write ops.

cpufreq: Don't allow updating inactive policies from sysfs
Later commits would change the way policies are managed today. Policies
wouldn't be freed on cpu hotplug (currently they aren't freed only for
suspend), and while the CPU is offline, the sysfs cpufreq files would
still be present.
User may accidentally try to update the sysfs files in following
directory: '/sys/devices/system/cpu/cpuX/cpufreq/'. And that would
result in undefined behavior as policy wouldn't be active then.
Apart from updating the store() routine, we also update __cpufreq_get()
which can call cpufreq_out_of_sync(). The later routine tries to update
policy->cur and starts notifying kernel about it.

intel_pstate: Force setting target pstate when required
During initialization and exit it is possible that the target pstate
might not actually be set. Furthermore, the result can be that the
driver and the processor are out of synch and, under some conditions,
the driver might never send the processor the proper target pstate.
This patch adds a bypass or do_checks flag to the call to
intel_pstate_set_pstate. If bypass, then specifically bypass clamp
checks and the do not send if it is the same as last time check. If
do_checks, then, and as before, do the current policy clamp checks,
and do not do actual send if the new target is the same as the old.

intel_pstate: change some inconsistent debug information
Commit
ce717613f3fb (intel_pstate: Turn per cpu printk into pr_debug)
turned per cpu printk into pr_debug. However, only half of the change
was done, introducing an inconsistency between entry and exit from
driver pstate control. This patch changes the exit message to pr_debug
also.
The various messages are inconsistent with respect to any identifier
text that can be used to help isolate the desired information from a
huge log. This patch makes a consistent identifier portion of the
string.

blk-mq: free hctx->ctxs in queue's release handler
Now blk_cleanup_queue() can be called before calling
del_gendisk()[1], inside which hctx->ctxs is touched
from blk_mq_unregister_hctx(), but the variable has
been freed by blk_cleanup_queue() at that time.
So this patch moves freeing of hctx->ctxs into queue's
release handler for fixing the oops reported by Stefan.

cfg80211: wext: clear sinfo struct before calling driver
Until recently, mac80211 overwrote all the statistics it could
provide when getting called, but it now relies on the struct
having been zeroed by the caller. This was always the case in
nl80211, but wext used a static struct which could even cause
values from one device leak to another.
Using a static struct is OK (as even documented in a comment)
since the whole usage of this function and its return value is
always locked under RTNL. Not clearing the struct for calling
the driver has always been wrong though, since drivers were
free to only fill values they could report, so calling this
for one device and then for another would always have leaked
values from one to the other.
Fix this by initializing the structure in question before the
driver method call.

iommu/vt-d: Change PASID support to bit 40 of Extended Capability Register
The existing hardware implementations with PASID support advertised in
bit 28? Forget them. They do not exist. Bit 28 means nothing. When we
have something that works, it'll use bit 40. Do not attempt to infer
anything meaningful from bit 28.
This will be reflected in an updated VT-d spec in the extremely near
future.

MIPS: pgtable-bits: Fix XPA damage to R6 definitions.

drm/i915: Fix DDC probe for passive adapters
Passive DP->DVI/HDMI dongles on DP++ ports show up to the system as HDMI
devices, as they do not have a sink device in them to respond to any AUX
traffic. When probing these dongles over the DDC, sometimes they will
NAK the first attempt even though the transaction is valid and they
support the DDC protocol. The retry loop inside of
drm_do_probe_ddc_edid() would normally catch this case and try the
transaction again, resulting in success.
That, however, was thwarted by the fix for [1]:

drm: give up on edid retries when i2c bus is not responding
This added code to exit immediately if the return code from the
i2c_transfer function was -ENXIO in order to reduce the amount of time
spent in waiting for unresponsive or disconnected devices. That was
possible because the underlying i2c bit banging algorithm had retries of
its own (which, of course, were part of the reason for the bug the
commit fixes).

drm/i915: use GMBUS to manage i2c links
we've been flipping back and forth enabling the GMBUS transfers, but
we've settled since then. The GMBUS implementation does not do any
retries, however, bailing out of the drm_do_probe_ddc_edid() retry loop
on first encounter of -ENXIO. This, combined with Eugeni's commit, broke
the retry on -ENXIO.
Retry GMBUS once on -ENXIO on first message to mitigate the issues with
passive adapters.

ntb: iounmap MW reg and vbase in error path
The MW regbase and vbase(s) were not being freed if an error occurred
in the vbase allocation loop. This is corrected by updating the error
path for the allocation loop to err4.

70008aa50e92 ("skbuff: convert to skb_orphan_frags") replaced
open coded tests of SKBTX_DEV_ZEROCOPY and skb_copy_ubufs with calls
to helper function skb_orphan_frags. Apply that to the last remaining
open coded site.

net: bcmgenet: power on MII block for all MII modes
The RGMII block is currently only powered on when using RGMII or
RGMII_NO_ID, which is not correct when using the GENET interface in MII
or Reverse MII modes. We always need to power on the RGMII interface for
this block to properly work, regardless of the MII mode in which we
operate.

ipv6: Fix protocol resubmission
UDP encapsulation is broken on IPv6. This is because the logic to resubmit
the nexthdr is inverted, checking for a ret value > 0 instead of < 0. Also,
the resubmit label is in the wrong position since we already get the
nexthdr value when performing decapsulation. In addition the skb pull is no
longer necessary either.
This changes the return value check to look for < 0, using it for the
nexthdr on the next iteration, and moves the resubmit label to the proper
location.
With these changes the v6 code now matches what we do in the v4 ip input
code wrt resubmitting when decapsulating.

ipv6: fix possible use after free of dev stats
The memory pointed to by idev->stats.icmpv6msgdev,
idev->stats.icmpv6dev and idev->stats.ipv6 can each be used in an RCU
read context without taking a reference on idev. For example, through
IP6_*_STATS_* calls in ip6_rcv. These memory blocks are freed without
waiting for an RCU grace period to elapse. This could lead to the
memory being written to after it has been freed.
Fix this by using call_rcu to free the memory used for stats, as well
as idev after an RCU grace period has elapsed.

It comes from the fact that devices can called some code from the DMA
controller with irq disabled. _bh variant is not intended to be used in
this case since it can enable irqs. Switch to irqsave/irqrestore variant to
avoid this situation.

bridge: disable softirqs around br_fdb_update to avoid lockup
br_fdb_update() can be called in process context in the following way:
br_fdb_add() -> __br_fdb_add() -> br_fdb_update() (if NTF_USE flag is set)
so we need to disable softirqs because there are softirq users of the
hash_lock. One easy way to reproduce this is to modify the bridge utility
to set NTF_USE, enable stp and then set maxageing to a low value so
br_fdb_cleanup() is called frequently and then just add new entries in
a loop. This happens because br_fdb_cleanup() is called from timer/softirq
context. The spin locks in br_fdb_update were _bh before commit
f8ae737deea1
("[BRIDGE]: forwarding remove unneeded preempt and bh diasables")
and at the time that commit was correct because br_fdb_update() couldn't be
called from process context, but that changed after commit:
292d1398983f ("bridge: add NTF_USE support")
Using local_bh_disable/enable around br_fdb_update() allows us to keep
using the spin_lock/unlock in br_fdb_update for the fast-path.

mpls: fix possible use after free of device
The mpls device is used in an RCU read context without a lock being
held. As the memory is freed without waiting for the RCU grace period
to elapse, the freed memory could still be in use.
Address this by using kfree_rcu to free the memory for the mpls device
after the RCU grace period has elapsed.

be2net: Replace dma/pci_alloc_coherent() calls with dma_zalloc_coherent()
There are several places in the driver (all in control paths) where
coherent dma memory is being allocated using either dma_alloc_coherent()
or the deprecated pci_alloc_consistent(). All these calls should be
changed to use dma_zalloc_coherent() to avoid uninitialized fields in
data structures backed by this memory.

lockdep: Fix a race between /proc/lock_stat and module unload
The lock_class iteration of /proc/lock_stat is not serialized against
the lockdep_free_key_range() call from module unload.
Therefore it can happen that we find a class of which ->name/->key are
no longer valid.
There is a further bug in zap_class() that left ->name dangling. Cure
this. Use RCU_INIT_POINTER() because NULL.
Since lockdep_free_key_range() is rcu_sched serialized, we can read
both ->name and ->key under rcu_read_lock_sched() (preempt-disable)
and be assured that if we observe a !NULL value it stays safe to use
for as long as we hold that lock.
If we observe both NULL, skip the entry.

amd-xgbe: Use disable_irq_nosync from within timer function
Since the Tx timer function runs in softirq context the driver needs
to call disable_irq_nosync instead of a disable_irq.

rhashtable: add missing import <linux/export.h>
rhashtable uses EXPORT_SYMBOL_GPL() without importing linux/export.h
directly it is only imported indirectly through some other includes.
